Evaluations for Children and Teens
Ages 5-17

Together, we’ll gain clear insight into your child’s strengths and challenges and how they impact their academic, social, and emotional well-being.

The Evaluation Process

Step 1

Schedule a Consultation

A 15-minute consultation to see if an evaluation is appropriate. This can be done by phone or virtually.

Step 2

Complete Intake Forms

Complete forms, including consent, developmental history, and current functioning. These forms can be done online or on paper.

Step 3

Do the Intake Interview 

As you know your child best, it is important to take the time to get to know all about your child. We will meet virtually for about an hour to discuss your concerns, review your child’s developmental history, and to develop goals for the evaluation.

Step 4

Participate in Testing

Your child will participate in formal and informal testing in the office. Testing is only done in person, it cannot be done virtually. Testing sessions are scheduled to 3 hour blocks. Typically two testing sessions are needed.

Step 5

Have Feedback Session

After testing is finished we will schedule a time to review the results of the evaluation and answer questions you may have. This can be done in person or virtually.

Step 6

Receive the Final Report  

Clients may leave the feedback session with the final report. If changes need to be made to the report based on information in the feedback session, the final report will be sent in about a week after the feedback session.
